Payment Options

We accept cash, check, Visa and MasterCard. Non-insured patients receive a 5% cash discount on day of service payment. Senior citizens (62 years and older) will receive an additional 5% cash discount on day of service payment.

Insurance

• Our office is in network with Cigna, Delta Dental Premier and Humana. As a courtesy, we will submit claims to most traditional insurances for our patients. Please contact your insurance provider with any questions regarding out of network coverage.

• We cannot submit claims for Medicare and Network Health. Payment will be due on the day of service. We will provide you a completed claim form to submit to your insurance carrier, they will reimburse you directly based on the structure of your policy.
